In order to participate in a sport, you will need to complete and turn in the following paperwork:

Physical and Participation Form (Signed by Medical Doctor)

Emergency Form (Blue Card)

Sports Fee

*All of these items can be submitted to the Main Office

**Hybrid Sports are a cross between an Intramural and Interscholastic. Games, meets and matches may be off site where transportation is required

WINTER SPORTS

Girls Basketball: October 23 – December 16

Boys Basketball: January 8 – March 2

SPRING SPORTS

Girls Volleyball: February 26 – April 27

Hybrid/Boys Volleyball: February 28 – April 22

To participate need a Current Physical, Emergency Blue Card, Participation Form and Sports Fee

Girls Golf (District Team): March 18 – April 28

To participate need a Current Physical, Emergency Blue Card, Participation Form and Sports Fee of $50 for greens fees

Boys Track and Field: April 3 – May 13

To participate need a Current Physical, Emergency Blue Card, Participation Form and Sports Fee

Co-Ed Soccer (Hybrid Sport): April 8 – May 13

To participate need a Current Physical, Emergency Blue Card, Participation Form and Sports Fee of $25
